All medical and liability release forms as well as a copy of everyone's insurance card must be submitted. Additionally, ALL ADULT LEADERS attending must read and follow all Youth Event Policies and submit Background Checks, Child Abuse Clearances, and FBI Criminal Background Checks to the Synod Office. Per new child clearance laws, please submit all background clearances in accordance to the new policy. Even if you have previously submitted yours in the past, please submit them electronically via scanned PDF document to djackson@sepa.org.

 Please Note: Please be sure to have all of your information to be entered before starting. For ticketing reasons the form will not take submissions if they are submitted after 15 minutes of starting the form.
